Home and Community Based Services
Who Is Eligible
- Elderly and physically disabled individuals.
- Medicaid-eligible or current Medicaid recipients.
- Residents of Silver Bow, Beaverhead, Deer Lodge, Granite, Powell, and Madison counties.
- Residents of the community living in their homes or in a community personal care facility.
- Pre-screening required by Mountain-Pacific Quality Health Foundation to determine level of care.
Program Description & Goals
Case Management is also known as Home and Community Based Services (HCBS) Medicaid Waiver. HCBS is a program under the U.S. Department of Health and Human Services Senior and Long term Care Division to provide in-home and community services to elderly and physically disabled people who would otherwise need to reside in a hospital or a long-term facility. The HCBS program is managed by the Case Management Team.
The program’s goals are:
To provide a person a choice of home or institution to meet their long–term care needs.
- Offering that person a less restrictive alternative.
- Enhancing the use of natural support systems
- Eliminating duplication of services through effective case management
- Ensuring that a person participates in developing their own plan of care.
Case Management Services
The case management team consists of a social worker and a registered nurse who:
- Develop an individualized plan of care with each client and their family; including services to be provided by the HCBS Program.
- Monitor and coordinate plans of care between clients, their families, providers, healthcare professionals and others involved.
- Authorize payment claims for all HCBS services.
- Identify community agencies, resources and services and make appropriate referrals as needed.
- Activate and maximize existing network of family and friends.
- Be an advocate for individuals as necessary.
